Additional Data Reporting

through the Special Education Web Portal

All school districts and 2R charter schools are required annually to report the following items through the Additional Data Reporting section of the Special Education Web Portal:

  • Parentally Placed Private School Students
    1. The number of parentally placed private school students who were referred for initial evaluation and whose evaluations were completed within a given time period.
    2. The number of students in a. who were determined eligible for special education.
    3. The number of parentally placed private school students who were referred for reevaluation and whose reevaluations were completed within a given time period.
    4. The number of students in c. who continued to be eligible after reevaluation.
    5. The total number of parentally placed private school students served with services plans during a given time period.
  • LEA district-wide assessments
    • Did the district administer district-wide assessments during the school year?
  • Special Education Forms and Policies and Procedures
    • Adoption/Revision of model special education forms
    • Adoption/Revision of model special education policies and procedures
